Centre injury/incident report form
For injuries where medical attention is sought or an ambulance is called, details should be entered on an insurance form. This is recommended to be done at the time of injury to start the process, even if there is later found to be insufficient grounds to submit a claim. (Note that ambulance fees should be claimed).
For injuries/incidents of a more minor nature, a record should be kept by the Club/Centre or LAWA (for LAWA Events) in an incident register.
